I hereby fully release and discharge Pet Friendly Services (PFS) its employees, directors, officers, agents, and liability insurance carriers, from all actions, damages or judgments which I have now or in the future against PFS, for all personal injuries to myself, known or unknown, and/or arising out of activities of myself as a volunteer of PFS. I, the undersigned, have read this release and understand all of its terms and I execute it voluntarily and with full knowledge of it significance. I further consent to the unrestricted use by HIS and/or person(s) authorized by them of any photographs, recordings, interviews, or videotapes taken of me while volunteering for the purpose of advertising, marketing or public relations.

Note: If you have Court Ordered Community Service, PFS does not allow volunteers with past histories of felonies, theft, sexual or violent crimes, or animal abuse/neglect related crimes.
